70대 초반 여자
종합병원에서 빈혈이 있다고 대장내시경 권유받음
본원 혈액 수치 11.1
빈혈의 평가로서 혹은 이전에 시행하지 않은 암 스크리닝으로서 대장내시경 시행
간만곡부에 16 mm 측방발육형 용종
주변으로 낮게 펴져나간 부분이 있어 경계 판단에 주의가 필요
piecemeal로 육안적 완전 절제
조직검사
[ DIAGNOSIS ]
1. Colon, hepatic flexure, colonoscopic biopsy;
Tubular adenoma (with low grade dysplasia)
2. Colon, hepatic flexure, colonoscopic polypectomy;
1) Villotubular adenoma with focal high grade dysplasia
(mostly low grade dysplasia with focal severe dysplasia)
* marginal status(resection margin): uncertain for evaluation (fragmented specimen)
2) Atypical cells* or clusters* having severe artifact in lamina propria (see note)
* DDX; a. Carcinoid tumor (neuroendocrine tumor)
b. Adenocarcinoma, poorly differentiated
[ NOTE ]
* Immunomarker study is needed for pathologic differential diagnosis.
2)소견에 대해 chromogranin, synaptophysin 시행해 볼 수 있겠다. 고등급 용종 이외에 특이 소견이 있어 3개월 추적 검사를 하는 것을 계획할 수 있겠다.
