처음에는 두드러기인가 하고 알레르기 관점의 질문; 집에 날다람쥐 (4개월, sugar glider), 과거에 새도 키우고 20대때..

"가렵다" 까지 들으면 한쪽의 국소 두드러기인가? 한쪽만 혈관부종과 같이 생겼나 하는 생각이 있었으나..
"찡그리면 아프다. 가만히 있으면 맞은듯한 ...욱씬.."

SUMMARY AND RECOMMENDATIONS — Preseptal cellulitis and orbital cellulitis involve different anatomic sites, with preseptal cellulitis referring to infections of the soft tissues anterior to the orbital septum and orbital cellulitis referring to infections posterior to it (figure 1). Orbital cellulitis involves the muscles and fat located within the orbit. Although preseptal and orbital cellulitis may be confused with one another because both can cause ocular pain and eyelid swelling and erythema, they have very different clinical implications. Preseptal cellulitis is generally a mild condition that rarely leads to serious complications, whereas orbital cellulitis may cause loss of vision and, very rarely, even loss of life. Orbital cellulitis can usually be distinguished from preseptal cellulitis by its clinical features (ophthalmoplegia, pain with eye movements, and proptosis) and by imaging studies; in cases in which the distinction is not clear, clinicians should treat patients as though they have orbital cellulitis. Both conditions are more common in children than in adults.
안와전방 연조직염과 안와 연조직염은 해부학적으로 다른 부위를 침범합니다. 안와전방 연조직염은 안와 격막 앞쪽의 연조직 감염을, 안와 연조직염은 안와 격막 뒤쪽의 연조직 감염을 의미합니다(그림 1). 안와 연조직염은 안와 내 근육과 지방 조직에 발생합니다. 두 질환 모두 안구 통증, 눈꺼풀 부종 및 발적을 유발할 수 있어 혼동하기 쉽지만, 임상적 의미는 매우 다릅니다. 안와전방 연조직염은 일반적으로 경미한 질환으로 심각한 합병증으로 이어지는 경우는 드물지만, 안와 연조직염은 시력 상실을 유발할 수 있으며, 매우 드물게는 사망에 이를 수도 있습니다. 안와 연조직염은 임상적 특징(안구 운동 마비, 안구 운동 시 통증, 안구 돌출)과 영상 검사를 통해 안와전방 연조직염과 구별할 수 있습니다. 구별이 명확하지 않은 경우에는 안와 연조직염으로 간주하고 치료해야 합니다. 두 질환 모두 성인보다 어린이에게 더 흔합니다.
●Preseptal cellulitis arises most commonly from sinusitis or a contiguous infection of the soft tissues of the face and eyelids secondary to local trauma, insect or animal bites, or foreign bodies.
안와전방 연조직염은 가장 흔하게는 부비동염이나 국소 외상, 곤충이나 동물의 물림, 또는 이물질로 인한 얼굴 및 눈꺼풀 연조직의 인접 감염에서 발생합니다.
●The most common causes of preseptal cellulitis are Staphylococcus aureus (including community-acquired methicillin-resistant S. aureus [CA-MRSA]), Streptococcus pneumoniae and other streptococci, and anaerobes (table 1).
안와전방 연조직염의 가장 흔한 원인은 황색포도상구균(지역사회 획득 메티실린 내성 황색포도상구균[CA-MRSA] 포함), 폐렴구균 및 기타 연쇄상구균, 그리고 혐기성균입니다(표 1).
●Patients with preseptal cellulitis typically present with unilateral ocular pain, eyelid swelling, and erythema (picture 1). During the initial evaluation, it is critical to distinguish preseptal cellulitis from the more serious orbital cellulitis (table 2 and table 3). Although both preseptal cellulitis and orbital cellulitis typically cause eyelid swelling and erythema, the presence of ophthalmoplegia, pain with eye movements, chemosis, and/or proptosis occur with orbital cellulitis, but not with preseptal cellulitis.
안와전방 연조직염 환자는 일반적으로 한쪽 눈의 통증, 눈꺼풀 부종 및 발적을 나타냅니다(그림 1). 초기 진찰 시에는 안와전방 연조직염을 보다 심각한 안와 연조직염과 감별하는 것이 중요합니다(표 2 및 표 3). 안와전방 연조직염과 안와 연조직염 모두 눈꺼풀 부종과 발적을 유발하는 것이 일반적이지만, 안구 운동 마비, 안구 운동 시 통증, 결막 부종 및/또는 안구 돌출은 안와 연조직염에서만 나타나는 증상입니다.
●The diagnosis can usually be made on the basis of these clinical features. However, in uncertain cases, computed tomography (CT) scanning of the orbits and sinuses can usually distinguish preseptal cellulitis from orbital cellulitis.
일반적으로 이러한 임상적 특징을 바탕으로 진단할 수 있습니다. 그러나 진단이 불확실한 경우에는 안와 및 부비동의 컴퓨터 단층 촬영(CT)을 통해 안와전방 연조직염과 안와 연조직염을 구분할 수 있습니다.
●Adults and children older than one year of age with mild preseptal cellulitis can be managed on an outpatient basis, provided that the patient has no signs of systemic toxicity and close follow-up is ensured. In these cases, we recommend treatment with oral antibiotics with activity against S. aureus (including MRSA) and streptococci, such as clindamycin monotherapy or combination therapy with trimethoprim-sulfamethoxazole plus one of the following agents: amoxicillin, amoxicillin-clavulanic acid, cefpodoxime, or cefdinir (Grade 2B). If the patient has not been immunized against Haemophilus influenzae, one of the combination regimens that includes a beta-lactam should be used.
경미한 안와전방 연조직염이 있는 성인 및 1세 이상 소아는 전신 독성 징후가 없고 면밀한 추적 관찰이 가능한 경우 외래에서 치료할 수 있습니다. 이러한 경우, 클린다마이신 단독 요법 또는 트리메토프림-설파메톡사졸과 다음 약제 중 하나(아목시실린, 아목시실린-클라불란산, 세프포독심 또는 세프디니르)를 병용한 요법과 같이 황색포도상구균(MRSA 포함) 및 연쇄상구균에 효과적인 경구 항생제 치료를 권장합니다(등급 2B). 환자가 헤모필루스 인플루엔자 예방 접종을 받지 않은 경우, 베타-락탐계 항생제를 포함하는 병용 요법을 사용해야 합니다.
●We generally recommend a treatment duration of five to seven days, but if signs of cellulitis persist at the end of this period, treatment should be continued until the erythema and swelling have resolved or nearly resolved.
일반적으로 5~7일간의 치료를 권장하지만, 이 기간이 지나도 봉와직염 증상이 지속될 경우 발적과 부기가 완전히 사라지거나 거의 사라질 때까지 치료를 계속해야 합니다.
●Patients younger than one year of age and those who are severely ill should be admitted to the hospital and managed according to the recommendations for orbital cellulitis. Patients with subtle clinical and/or radiographic findings suggesting that the orbit is involved, as well as young children who are not able to cooperate for a thorough examination, should also be treated as though they have orbital cellulitis given the serious complications of this entity.
1세 미만의 영유아 및 중증 환자는 입원하여 안와 봉와직염 치료 지침에 따라 치료해야 합니다. 임상적 또는 방사선학적 소견이 미미하여 안와 침범이 의심되는 환자, 그리고 철저한 검사에 협조하기 어려운 어린 소아의 경우에도 안와 봉와직염의 심각한 합병증을 고려하여 안와 봉와직염으로 간주하고 치료해야 합니다.
