상기 환자 dur을 통해서 응급실에서 항생제 투약후 퇴원한 것으로 알고 있었음.

혈압/고지혈 투약위해 약 3주뒤 내원

응급실 이후에 외래에서도 3일정도 항생제 투약했다고 하며.. 천천히 좋아졌다고 함, 6일 투약으로 충분?

CT는 응급실에서 시행한거 같고

최근 조직검사 (아마 세침검사, FNA) 하여 어제 결과를 들었다고 함, 금년말 수술 가능했으나 내년 1월로 계획 잡았다고

선천성 뭐라고 설명을 하는데... 암이 될수도 있어서? 뼈도 일부 잘라야 한다고...

작으면 이렇게 보일수 있는데 환자 사례는 크고 내부의 면이 복잡하였다.


감염이 되면 벽이 두꺼워지거나 내부에 찌꺼기의 생성


Thyroglossal duct cysts (TGDC) are the most common cause of midline neck masses. TGDC are typically located inferior to the hyoid bone (65%) in the region adjacent to the thyrohyoid membrane. However, these remnants can occur anywhere along the path followed by the primordial thyroid gland during descent from the base of the tongue. Frequently presenting as an asymptomatic neck mass in the pediatric population, the most common presentation in adults is underlying infection of the cyst. Other common causes of midline neck masses include lymphadenopathy, dermoid cysts, and various odontogenic anomalies. Classic physical exam findings include a mobile neck mass that moves with swallowing or protrusion of the tongue. Accompanying symptoms include sore throat, pain, dysphagia, hoarseness, and globus. Serious complications involve airway obstruction precipitated by rapid enlargement of the cyst. Findings on CT include a well-circumscribed lesion with significant rim enhancement.

갑상설관 낭종(TGDC)은 목 정중앙에 발생하는 종괴의 가장 흔한 원인입니다. TGDC는 주로 설골 하방(65%), 즉 갑상설골막(thyrohyoid membrane) 인접 부위에 위치합니다. 그러나 이러한 잔존 조직은 원시 갑상선이 혀뿌리에서 하강하는 경로를 따라 어느 곳에서든 발생할 수 있습니다. 소아에서는 주로 무증상 목 종괴로 나타나는 반면, 성인에서는 낭종의 감염이 동반된 형태로 나타나는 경우가 가장 흔합니다. 목 정중앙 종괴를 유발하는 다른 흔한 원인으로는 림프절병증, 유피낭종(dermoid cyst), 그리고 다양한 치성 기형 등이 있습니다. 전형적인 신체 검진 소견은 연하 운동이나 혀를 내밀 때 함께 움직이는 가동성 목 종괴입니다. 동반 증상으로는 인후통, 통증, 연하 곤란, 쉰 목소리, 그리고 인후 이물감(globus) 등이 있습니다. 심각한 합병증으로는 낭종의 급격한 팽창으로 인한 기도 폐쇄가 있습니다. CT 소견상으로는 뚜렷한 경계를 보이며 가장자리가 뚜렷하게 조영 증강되는 (초음파에서 고음영의 염증성 지방변화가 이렇게 보일거 같다.) 병변이 관찰됩니다.

Definitive treatment of infected TGDC involves both antibiotics and needle aspiration. Examination of the aspirate allows for identification of the involved organisms as well as cytologic analysis to rule out underlying TGDC carcinoma. The most common organisms involved include Staphylococus epidermis, Haemophilus influenza, and Staphylococcus aureu. Following control of the underlying infection, the patient may elect to surgically remove the cyst to prevent further recurrence.

감염된 갑설관 낭종(TGDC)에 대한 확립된 치료법은 항생제 투여와 바늘 흡인술을 병행하는 것입니다. 흡인물을 검사하면 원인균을 파악할 수 있을 뿐만 아니라 세포학적 분석을 통해 기저 질환으로서의 TGDC 암종 가능성을 배제할 수 있습니다. 흔히 관여하는 원인균으로는 표피포도상구균(Staphylococcus epidermidis), 인플루엔자균(Haemophilus influenzae), 황색포도상구균(Staphylococcus aureus) 등이 있습니다. 기저 감염이 조절된 후, 환자는 재발을 방지하기 위해 낭종을 수술로 제거하는 방법을 선택할 수 있습니다.



갑상설관낭종과 암의 연관성; 1% 정도로 낮고 갑상선 조직 기원의 암이 발생

The overall risk of a thyroglossal duct cyst (TGDC) becoming malignant is very low, occurring in approximately 1% of cases. While these congenital neck masses are the most common midline anomalies found in children and young adults, the vast majority are entirely benign. [1, 2, 3, 4]

When a malignancy does occur, it is usually diagnosed incidentally through histopathological examination after the cyst has already been surgically removed. [1, 2]

Breakdown of Cancer Types

If a malignancy develops within a thyroglossal duct remnant, it most commonly originates from ectopic thyroid tissue trapped within the duct during embryonic development. [1, 2, 3, 4]

  • Papillary Thyroid Carcinoma (PTC): This is by far the most frequent type, accounting for 75% to 90% of all malignant cases.

  • Squamous Cell Carcinoma: Occurs in about 5% of cases and arises directly from the epithelial lining of the cyst.

  • Follicular Thyroid Carcinoma: A rarer variant, appearing in roughly 1% to 2% of cases.

  • Anaplastic Carcinoma: Extremely rare (less than 1%), carrying a much more aggressive prognosis. [1, 2, 3]

Clinical Red Flags & High-Risk Factors

While most malignant cysts look and feel exactly like benign ones, clinicians watch for certain high-risk indicators that elevate the suspicion of cancer: [1, 2, 3, 4]

  • Sudden or Rapid Growth: A quick increase in the size of a long-standing, persistent midline neck mass.

  • Physical Characteristics: The mass feels hard, fixed to surrounding tissues, or irregular rather than smooth and movable.

  • Associated Symptoms: Onset of pain, difficulty swallowing (dysphagia), or changes in voice (dysphonia).

  • Patient Demographics: While benign cysts are usually found in childhood, malignant transformations are more commonly diagnosed in adults, with a mean age of presentation around 40 years old. [1, 2, 3, 4, 5, 6]

Preoperative Detection Challenges

Preoperative diagnosis is notoriously difficult. Fine-Needle Aspiration Biopsy (FNAB, 세침검사) yields highly variable accuracy (often only positive in 66% of cases) because the fluid inside the cyst dilutes the sample, leading to low cellular numbers. [1, 2, 3]

However, advanced imaging can provide clues. On a CT scan or ultrasound, the presence of mural nodules, solid enhancing components, or microcalcifications inside the cyst wall strongly points toward potential malignancy. [1, 2, 3]

Treatment and Prognosis

The standard treatment is the Sistrunk procedure, which involves removing the cyst, the central portion of the hyoid bone (hyoid 뼈의 제거), and the tract up to the tongue base. [1, 2]

  • Low-Risk Patients: The Sistrunk procedure alone is highly curative for localized, low-risk papillary carcinomas. [1, 2]

  • High-Risk Patients: If the patient has concurrent thyroid nodules (there is a 27% to 56% chance of synchronous thyroid cancer), lymph node involvement, or a tumor size larger than 1.5 cm, doctors may recommend a total thyroidectomy and postoperative radioactive iodine therapy. [1, 2]

  • Prognosis: For the vast majority who develop the common papillary variant, the long-term outlook is excellent, boasting a 10-year overall survival rate of over 95%. [1, 2]​




Around 30 % of patients present with a recurrent or previous history of infected cyst (Foley and Fallat, 2006). Because of the anatomical association with the oral cavity, thyroglossal duct cysts are at risk of infection as reflected by the common type of infecting organisms, namely, Staphylococcus aureus, Staphylococcus epidermidis, and Haemophilus influenza (Acierno and Waldhausen, 2007). Unusual organisms have been reported as well, including Neisseria gonorrhea (Wongrakpanich et al., 2015); Coccidioides spp. (Nelson et al., 2007), and N. meningitidis (Gupta et al., 2008).

환자의 약 30%는 낭종 감염이 재발했거나 과거에 감염된 병력이 있는 것으로 나타납니다(Foley and Fallat, 2006). 구강과 해부학적으로 연관되어 있어 갑상설관 낭종은 감염 위험이 있는데, 이는 황색포도상구균(Staphylococcus aureus), 표피포도상구균(Staphylococcus epidermidis), 인플루엔자균(Haemophilus influenzae)과 같은 흔한 원인균에서 잘 드러납니다(Acierno and Waldhausen, 2007). 또한 임균(Neisseria gonorrhoeae)(Wongrakpanich et al., 2015), 콕시디오이데스(Coccidioides spp.)(Nelson et al., 2007), 수막구균(N. meningitidis)(Gupta et al., 2008) 등 드문 원인균에 의한 감염 사례도 보고된 바 있습니다.
