어제 오른쪽이 심해지고.. 잘때도 불편..

머리속에 떠오른 단어는 화농성 한선염, Hidradenitis suppurativa

Hidradenitis suppurativa is a chronic inflammatory skin condition characterized by painful lesions such as deep-seated nodules, abscesses, skin tunnels, and fibrotic scars.

화농성 한선염은 깊숙이 자리 잡은 결절, 농양, 피부 터널, 섬유성 흉터와 같은 통증성 병변을 특징으로 하는 만성 염증성 피부 질환입니다.

지인인 피부과 선생님 말씀이 ablation 하면 된다는데.... 만성 염증성 질환에서 간단한것 인지는 모르겠다.

Hidradenitis suppurativa (HS) ablation involves using specialized lasers or energy devices, most commonly CO2 lasers, to vaporize diseased tissue, tunnels, and hair follicles, promoting healing by secondary intention. This minimally invasive technique is effective for severe, chronic lesions, offering long-term remission, reduced recurrence compared to traditional surgery, and faster recovery times (often 4-5 weeks).

Key Ablation Methods for HS

  • CO2 Laser Ablation/Deroofing: This is the gold standard for laser treatment, where the surgeon uses a CO2 laser to "deroof" (open) sinuses and tunnels (sinus tracts) while vaporizing damaged tissue. It is highly effective for chronic, recurring abscesses and tunnels, often performed under local anesthesia.

  • Diode Laser (1470 nm): These lasers penetrate deeper to target inflamed, diseased tissues, sebaceous glands, and hair follicles, making them useful for deep, nodular formations.

  • Microwave Ablation (MWA): Similar to miraDry, this technology uses electromagnetic waves to thermolyze hair follicles and apocrine glands in the dermis. It is being explored for its potential to prevent future HS lesions by destroying the follicles from which they start.

Key Considerations

  • Recovery: Procedures are usually done under local anesthesia, allowing for quick recovery, though healing can take 4-8 weeks.

  • Benefits: Reduced need for extensive surgery, less pain, and improved quality of life.

  • Risks/Side Effects: Potential for local skin reactions (redness, swelling), pain, and changes in skin pigmentation (especially in darker skin tones).

  • Not a Cure: While highly effective, HS is chronic, and new lesions can appear, requiring maintenance treatments. 완치는 아니다!
