진료실에서 혈압 측정하려하니 빠른 빈맥이 청진되어 위에 기록과는 다른 문제가 있는것이 확실

4년전 **대학교 순환기내과에서 cryoballon ablation 시술 받은듯 기술 - 심방세동에 대해
심전도는 아래와 같은데, 약 150회 이상이며, 자동 판독은 정율동 빈맥이라고 나오나
얼핏보면 130~150 이상의 규칙적인 빈맥으로 상심실성빈맥인가 하였으나...
V2 lead를 확대해서 보면 규칙적인 p wave가 약 300회/분 속도로 관찰된다.

2. Hemodynamically Stable Patients
A. Rate Control (The First Step) 속도조절
If the heart rate is too fast (typically 150 bpm in a 2:1 conduction), medications are used to slow the conduction through the AV node. 특히 2:1 블록과 같이 속도가 빠를때
Beta-blockers: IV Metoprolol or Esmolol (short-acting, easy to titrate).
Calcium Channel Blockers: IV Diltiazem or Verapamil.
Note: Atrial flutter can be more "obstinate, 완강하게 버티는" to rate control than AFib, often requiring higher doses; 심방세동보다 조절하기가 어렵다.
B. Rhythm Control (Restoring Sinus Rhythm) 리듬조절
Pharmacological Cardioversion:
Ibutilide (IV): (As we discussed, not commonly available in Korea, 국내에 출시된 제품은 없다고 한다.). It is highly effective for AFL but requires 4 hours of ECG monitoring due to the risk of QT prolongation.
Amiodarone (IV): Used if the patient has structural heart disease or heart failure, though it is slower to act.
Vernakalant (IV): An option in some regions for rapid conversion.
Overdrive Pacing: If the patient has a pre-existing pacemaker or epicardial wires (e.g., after cardiac surgery), the device can be used to "pace out" the flutter without needing electricity or sedation.
4. Long-term Solution: Radiofrequency Ablation - 장기적 계획은 전극도자술
While not "acute," Catheter Ablation is now considered the first-line definitive treatment for typical Atrial Flutter. It has a success rate of over 95% and prevents future acute episodes much more effectively than long-term medication. 급성/응급이 아닐때는 전극도자술을 일차치료로 계획
