30대 후반 남자, 초진

​

접수; 벌레.... 물리고 세균감염 상담

3일전 저녁에 물렸다.

2일전 부터 간지럽고 부어오르고

진행하는 양상이라고 하며...

팔에 열감/욱씬거림...

눌러서 압통은 없으며, Td (-)

​

관찰함; 하박에 물려서 붉은띠 형색으로 겨드랑이까지 올라간 모습. 혈관의주행방향은 아닌... (사진)

약간 단단한 (edematous) 피부양상, 특히 물린곳에 가까운곳은

​

약 3/4 바퀴 돌아서 겨드랑이까지 발적이 띠 형태로 올라가는 모습

진료보면서 처음보는데...

​

병변 기술 (linear erythema bug bite) 을 통한 검색을 통해 아래와 같은 비슷한? 모습을 확인할수 있었다.

알레르기성인지, 중복된 세균성 감염도 있는지 구별이 필요한데 자주보는 상황이 아니고 물린뒤 늦게 발생하는 것으로 판단하여 봉와직염에 준해서도 처방하고 추적하기로 함

2일뒤 재방문은 하지 않음. 호전되었을 것으로 추정된다.

아래 표에 맞춰보면 경계는 명확하지 않았던거 같다. 그러나, 2일뒤 재방문하지 않은점 (호전) 을 고려하면 알레르기성 반응에 가까운 것으로 생각된다.

When to Seek Immediate Medical Evaluation

Because bacterial lymphangitis can progress rapidly into deeper tissue infections or enter the bloodstream, you should be evaluated by a healthcare professional immediately if you observe any of the following symptoms:

  • The red line is expanding rapidly up your arm or leg.

  • The bite site or streak becomes painful, throbbing, or extremely tender to touch.

  • You develop a fever, chills, or feel generally unwell.

  • You notice swollen, painful lumps in your groin or armpits (nearest lymph node clusters). [1, 2, 3, 4, 5]
