허리 통증과 일측 간지러움과 화끈거림, 힘을 줄때 한쪽 배가 나온다. 대상포진에 의한 감각, 운동 신경 증상 (pseudohernia)
40대 중반 남자
"어제부터 오른쪽 배가 붓는다. (실제로 배가 나온다)
오늘부터 화끈거린다." 를 주소로 내원
1주일 전에 허리가 아팠고.. 주로 밤에.. 근육통 양상?
5일전부터 아래 피부의 병변이 발생하고 간지러웠다가 기술; T10 높이의 오른쪽 피부분절, dermatome
; 발적을 동반한 papule양상으로 병변의 개수는 많지 않으나 증상/분포 및 모양에서 대상포진으로 진단함
배가 나온다고 하여 기립하여 배에 힘을 줘보면 아래와 같다.
초음파에서 신장 및 상행결장을 확인하였으며 특별히 종괴는 없었다.
복벽의 근막 (aponeurosis) 은 연속성을 잘 유지하고 있다.
그러나 복압을 높였을 때 근육의 두께에서 차이를 확인할 수 있다.
What is a pseudohernia? It is a limited protrusion of the abdominal wall without an evident muscle or aponeurotic defect. Relaxation of the anterior abdominal wall will cause it to bulge with increased intra-abdominal pressure. 가성탈장이란? 복압을 상승시킬때 복벽에 힘이 들어가지 않고 이완되면 튀어나오게 된다.
