소견은 유두암에 가까우나 수년간 크기 변화는 없는/그러나, 기관지에 접한... 세침검사에서 Bethesda III --> V - 동대문구 답십리, 전농동, 우리안애 우리안愛 내과

60대 후반 여자, 2019년 2월부터 고혈압/고지혈/골다공증 등 투약중

2019년에는 인지하지 못했으며

20년 초음파에서 협부에 가까운 우옆의 아래 경계에 아래와 같은 저음영, 경계는 불규칙하며 석회화가 동반된 갑상선 결절의 발견, 크기는 0.45 cm

; 모습도 걱정되는 소견이며 게다가 주변장기 (기관지) 에 접해 있다.

2021년 관찰

2022년은 검사하지 않았으며 23년도 검사; 크기는 거의 같으나 좀더 기관지에 맞다아 밀고 있는 느낌이 있어 세침검사 권유하여 시행

[ Cytologic Diagnosis ]

Thyroid, isthmus, 5.1x5.4cm, aspiration cytology ;

Atypia of Undetermined significance (Bethesda III)

(Several degenerated, enlarged, atypical cells with irregular nuclear membrane)

III에 해당하니 재검이 원칙인데... 상급병원에서 수개월뒤 세침검사위해 방문하거나, 1년 추적하기로 하였다.

2024년에도 결절의 크기는 큰 변화가 없는데...


작은 결절에 대한 세침검사가 targeting하기가 쉽지는 않다.


상급병원 진료 보기로 하여 2025년 5월에 외래를 보았으며 세침검사 재검

이번에는 Bethesda V 가 나온것으로 보인다

대학병원 주치의는 추적 권유도 하였다고 하나 가족들의 의사를 반영하여 수술 결정했다고 한다.


Prior studies have shown that active surveillance is safe in papillary thyroid cancers measuring 1 cm or smaller and confined to the thyroid without any lymph node involvement or spread outside of the neck.

크기는 작으나 기관지에 접해 있는 점을 고려한다면...

기관지를 침범했을 때

Tracheal invasion is a poor prognostic factor in well-differentiated thyroid cancer. Appropriate resection can improve the prognosis and maintain the patient’s quality of life. Shaving resection for superficial tracheal invasion is minimally invasive because it does not involve the tracheal lumen, despite the problematic risk of local recurrence. Window resection for tracheal mucosal and luminal invasion provides good tumor control and does not cause postoperative airway obstruction; however, the need for surgical closure of the tracheocutaneous fistula is a disadvantage of this method. Circumferential (sleeve) resection and end-to-end anastomosis are highly curative, but the risk of fatal complications, such as anastomosis dehiscence, is a concern.

대학병원에서 Bethesda III --> 본원에서 V

PTCN, 일부 낭성변화를 동반한 결절일때도 악성일 수 있다.
