결절성 하시모토 갑상선염 배경에서; 저음영 부분.. 세침에서 주로 휘틀 (Hurthle) 세포만 - 동대문구 답십리, 전농동, 우리안애 우리안愛 내과

상기 환자 5개월 지나 경동맥 초음파 위해 내원, 1년전 경화반이 있었다고....
경동맥 내막은 1.2 mm 정도였으며...
30년전에는 갑상선 항진증? 이다가 하시모토 갑상선염이라고 하는데... (일시적인 항진증 가능성)
경도맥 보면서 같이 관찰한 갑상선
종대와 함께 전반적으로 어두운 모습; 하시모토 갑상선염의 모습으로 볼 수 있으며...
저음영의 경계가 보이는 여러개의 결절, 우엽
좌엽
기관지 근처에 결절의 경계가 보이는데 아래쪽에 경계가 불분명한 저음영의 부분이 겹쳐서 혹은 튀어나오는 듯한 모습이 관찰됨
??? 부분도 저음영으로 보이는데..
가운데 등음영 부분이 있는 .. 우선 크기는 작고..
다른 각도로 관찰하면 결절내에서 저음영 부분이 아래쪽으로 밀고 나오는듯 하다. nodule on nodule
검사때 인지가 잘 되지 않았으나 아래쪽으로도 저음영 부분이 있기도 하다.
등음영 결절에서 자라나오는 듯한 저음영 결절은 무시할수 없다고 생각하여 세침검사를 상의하여 결절
하시모토 갑상선염이 갑상선 유두암과 연관있다고도 하며
다음과 같이 가장 흔하고, 뚜렷하게 유두암과 같은 암을 생각하며 세침검사를 한 경우가 아니다.
하시모토 갑상선염에 특징적인 림프구성 세포보다는 거의 전부가 휘틀세포라고 보고되고 있다. Bethesda III을 주긴했으나.. 다수의 결절일때 휘틀세포 과증식이 될수 있으나 휘틀종양 (bethesda IV) 를 배제할 수 없다고 기술되어 있다. 균일하지 않은 결절에서 아래의 세포 결과면 구별을 위한 수술?
[ Cytologic Diagnosis ]
Thyroid, right, 6.1x10.3mm, aspiration cytology ;
a) Atypia of Undetermined significance (with Hurthle cells) (Bethesda III) ( see note)
b) Background of thyroiditis
- The specimen is moderately cellular and consists almostly of Hurthle cells. Colloid is
scant.
Note: In a patient with multiple nodules, the findings likely represent a Hurthle cell hyperplas
ia in the setting of multinodular goiter, but a Hurthle cell neoplasm (Bethesda IV) cannot be en
tirely excluded.
* Follow-up cytology or surgical intervention, if clinically indicated (large size, etc.) is rec
ommended.

Bethesda III 이므로 추적 세침검사를 손을 바꿔 (상급병원) 계획해 보는 것이 좋겠다. 좀더 경험이 많은 내분비 종양 전문의의 이차의견을 위해서도...
의뢰전 다른곳의 저음영 부분의 관찰을 다시 고려... 세침검사한 곳과 유사한지...
휘틀세포가... 뚜렷한 구별이... 쉽지가 않은... 소견

Hürthle cell in thyroid aspirates is seen in variety of lesions such as Hashimoto's thyroiditis, multinodular goitre, Hürthle cell neoplasms (adenoma/carcinoma), head and neck irradiation, systemic chemotherapy, oncocytic variant of medullary, papillary carcinoma and long-standing Graves disease. Several studies have reached contradictory conclusions regarding the value of specific cytologic findings in these lesions.
Out of the 14 features, non-macrofollicular architecture, absence of background colloid (colloid 성분이 없는), absence of chronic inflammation (염증세포가 없는) and presence of transgressing blood vessels were statistically significant in predicting Hürthle cell neoplasm in 86% of Hürthle cell lesions.
Malignancy risk for solitary and multiple nodules in Hürthle cell–predominant thyroid fine-needle aspirations: A multi-institutional study
The overall risk of malignancy for HUR was 27%, whereas the risk of malignancy for AUS was 10%.
Given this lack of specificity of Hürthle cell metaplasia, the accurate diagnosis of Hürthle cell–predominant fine-needle aspiration (HC FNA) specimens can be extremely problematic. 특이도가 떨어지므로 휘틀세포 결과만으로 진단하기는 극도로 어렵다.

유두암 소견과 거리가 멀뿐...
대학병원에서 세침하지 말고 (waste of money라고 표현하며) 수술하자고 했다고 하며
외국인은 waste of money란 표현이 맘에 들지 않았는지 다른 대학병원으로 이차의견을 위해 이동하였다.
Bethesda II 양성 소견이었던 경우, 바로 위 경우와 초음파 소견만으로 차이를 두기에는...
