30대 중반 여자

오늘 새벽부터.. 쓰리다가를?? 반복...

왼쪽배가 아프다. 쥐어짜듯.. 살짝.. 지속.. 수시간?

구역감..

증상을 듣고 촉진을 하려는데..

누워서 본인이 눌러보는데 "윽" 할정도의 모습

구조적 소견이 있을 복통의 모습인데..

촉진시 배꼽아래 하복부 양측에 모두 뚜렷하고 민감한 압통

반발압통은 없었다.

발생의 속도와 이정도 압통이 한쪽이라면 출혈성 낭종도 배제해야하는데..

압통은 양측에 위쪽 경계가 배꼽까지 상당히 높았다.

방광위쪽에 염증성 지방변화

우측 배꼽 바로아래까지의 분포, 압통범위의 경계와도 유사

맹장, cecum에서 나오는 확장된 관상구조는 없었으며, 충수돌기염의 배제

골반 위쪽 하복부의 양측.. 전반적인 염증성 지방변화

자궁과 난소는 복부초음파로는 소장의 음영에 가려 거의 관찰되지 않음.

좀더 아래 방광까지 염증성 지방변화가 연속

임상적으로 골반에 국한된 복막염으로, 부인과적 골반염에 무게를 두어 감별을 위해 산부인과 진료 안내

증상의 심한 정도에서 응급실을 통해서...

Radiographic features, 골반염

Imaging features 영상의학적 소견은 are often non-specific (비특이적) but are disproportionate to what may be apparent from symptoms 증상과 비례하지도 않는다. If imaged early (e.g. during the cervicitis stage), there may be no finding 자궁경부염만 있을때는 아무 소견도 없으며... If imaged very late, there may be an adnexal mass-like region with surrounding inflammatory change, and the fallopian tube and ovary may not be distinguished 아주 후반기엔 난소에 종괴같은 병변과 주변부 염증성 변화, 난관과 난소는 구별이 어려워진다..

Other associated findings include 7:

soft-tissue stranding and infiltration of pelvic floor fascial planes
thickening of uterosacral ligaments

Ultrasound

Ultrasound is usually the first imaging requested in a case of lower abdominal pain.

Early findings in PID include 12 : 초기소견

indistinct uterine margins 자궁의 경계가 흐지부지..
echogenic pelvic fat 골반의 염증성 지방변화
fallopian tube thickening 난관의 두꺼워짐

Few non-specific findings include 8: 비특이적인 몇몇의 소견

fluid in cul-de-sac 복수, culdesac에
fluid in endometrial cavity 자궁내 액체
increased ovarian volumes 난소의 비대?
increased thickness and vascularity of the endometrium 자궁내부 비대 및 혈류의 증가

In the most severe cases (가장심한경우), ultrasound may show adnexal masses with a heterogeneous echo-pattern.

Some sonographic signs associated with tubal inflammation include:

thickened/dilated fallopian tubes 난관이 비대해지고 확장
incomplete septa in the tube 난관에 격벽형성
fluid collection within the tubes (hydrosalpinx) 난관에 액체저류
increased vascularity around the tube on color Doppler 난관 주변 혈류 증가
the fat around the tube may be echogenic and there may be a small amount of reactive free fluid in the pelvis 난관주변 지방의 음영증가 및 소량의 복수
echogenic fluid in the tube (pyosalpinx) 난관에 지저분한 액체
results from adhesions causing tubal obstruction
