3개월전 산부인과 병원에서 검진

rGTP가 139 로 상승, AST/ALT는 정상범위

"당시 한약 복용중이었다.", "이후에 중지한 상태이다."

그외 공복 혈당 120

; 간초음파/갑상선 초음파 시행하였다.

수년전 출산시 항진증으로 1년 투약

최근 출산 이후 10개월 지난 시점

흑마늘 엑기스에 의한 독성간염, 소량의 복수


과거 알콜장애로 추정되는 분으로 rGTP/ALP가 같이 상승되어 있던 경우


rGTP만 상승된 경우 뚜렷한 설명이 어려운 경우도 있다.

rGTP만 상승한 경우; ISOLATED GAMMA-GLUTAMYL TRANSPEPTIDASE (GGT) ELEVATION — Elevated levels of serum GGT have been reported in a wide variety of clinical conditions, including pancreatic disease, myocardial infarction, renal failure, chronic obstructive pulmonary disease, diabetes mellitus, and alcoholism. High serum GGT values are also found in patients taking medications such as phenytoin and barbiturates. GGT is sensitive for detecting hepatobiliary disease, but its usefulness is limited by its lack of specificity. 특이성이 떨어지며 간이외에 다양한 임상적 상황에서 상승할 수 있다.

An elevated GGT with otherwise normal liver biochemical tests (including a normal alkaline phosphatase) should not lead to an exhaustive work-up for liver disease. We suggest GGT only be used to evaluate elevations of other serum enzyme tests (eg, to confirm the liver origin of an elevated alkaline phosphatase or to support a suspicion of alcohol abuse in a patient with an elevated AST and an AST to ALT ratio of greater than 2:1). 다른 간수치가 정상이면서 (ALP 포함) rGTP만 상승한 경우 너무 간질환에 대한 평가를 소모적으로 하지는 않아야 겠다. ALP가 상승한 경우 rGTP가 상승되어 있다면 ALP가 간질환에 의한 것으로 판단할 수 있으며, AST/ALT 비율이 2:1 이상으로 상승되어 있으면 rGTP가 술의 남용을 시사하는 검사로서 사용될 수 있겠다. ​
