당뇨 신경병증에 capsaicin 효과 평가, TCA, 삼환계 항우울제 약물에 의한 다한 증상
상기 환자 이후에
acetaminophen, tramadol 복합제가 도움이 되어
처방 1주일 후 잠에서 깨는 횟수가 3~4회에서 1~2회로
2주일후 30% 정도 호전된 상태
tramadol 진통제 처방 중지했더니 "뜨겁지는 않은데.. 따끔거린다."
2주 acetaminophen/tramadol 다시 추가하고 nortriptyline 은 하루 3번으로 복용 증량
; 다음 외래시 capsaicin 크림을 시도해보기를 계획함
2주뒤 내원
"요새 땀이 똑똑 ... 배쪽에 더 그렇다."
TCA 부작용 고려, 지속되는지 관찰이 필요

가장 심했을 때가 10 이면 6 정도 증상으로 40% 호전, "크림이 도움이 된다."
"손바닥, 손등, 발바닥, 발등에 바르고 있다. 증상이 없는 곳에 닿으면 자극이 된다."
; tramadol은 추후 줄여보기로 하고,
thioctic acid, nortriptyline (TCA계통), capsaicin 연고를 유지하면서 증상 조절 정도를 평가하기로 하였다.
추후 TCA + pregabalin 의 필요성 및 단일제제보다의 유용성은??
Capsaicin — Capsaicin is a naturally occurring component of many hot peppers and causes analgesia through local depletion of substance P. It is available as a cream and other formulations (gel, liquid, lotion, and patch) for topical application. In randomized trials in patients with painful diabetic neuropathy, capsaicin has been associated with modest but statistically significant improvement in pain compared with placebo.
We add capsaicin cream (0.075 percent applied topically four times daily) for patients with symptomatic painful diabetic polyneuropathy who are refractory to or intolerant of antidepressants (eg, amitriptyline, duloxetine, venlafaxine) or anticonvulsants (eg, pregabalin) discussed above. Local burning and skin irritation can occur, but this becomes less of a problem with continued use. Nevertheless, many patients are unable to tolerate the local burning pain, which is exacerbated by contact with warm water and hot weather.
Side effects - Antidepressants
Tricyclic antidepressants (TCAs); 발한 (특히 밤에) excessive sweating (especially at night)
